Transitional Housing program offers the opportunity for 17 homeless men and women to live in a sober living environment for up to 24 months while receiving intensive case management services to address issues of substance abuse, mental health, health concerns, education, training, employment and accessing income benefits and permanent housing.

Areas of service:  Temporary housing, Transportation-Bus pass, Intensive Case Management services, three meals per day, hygiene products, In-House Life skill classes, substance abuse AA/NA classes

Clinica Sierra Vista is one of the largest comprehensive migrant/community health center systems in the state including services such as transitional housing for the homeless, for homeless veterans and chronically homeless individuals, persons living with HIV, that are linked to housing or at-risk of losing housing permanent housing in collaboration with the Housing Authority and mental health and substance abuse outpatient services.

Areas of service: Housing and Emergency Shelter, Medical, Food and Clothing, Utilities, Veterans, Disability Resources, Transportation- Bus pass, Mental Health and Substance Abuse.

Clinica Sierra Vista will provide street outreach to identify and enroll homeless veterans/chronically homeless individuals into the programs for Mental Health and Substance Abuse services. Services are based on the Matrix Model for intensive outpatient alcohol and drug treatment.  Requirements for enrollment are referral form, verification of chronically/homeless or veteran, verification of housing voucher going into housing or at risk of losing housing.
